for The Broomfield Senior Resource Scholarship Program
Eligibility:
Must
be a resident of City & County of Broomfield
Recipient must be age 60 or older
Financial
Assistance is available for:
¢
Prescription drug cost and co-payments
¢ Fees for doctor's/dentist's visits
¢ CO-Payments for medical/dental treatment
¢ CO-Payments for Emergency Ambulance Transportation
¢ Durable Medical Equipment
¢ Other requests for medical assistance will be considered on an
individual basis
¢ Congregate meals and Meals-on-Wheels - up to 50%
¢ Easy Ride transportation services - up to 50%
¢ Other emergency situations
To Apply:
Request
an application from a Senior Center Staff Member at 280 Lamar Street,
Broomfield.
Applications for assistance are submitted on a monthly basis to the
Senior Resource Scholarship Committee for review and approval.
There
is a $300 per person assistance limit per year.
